Job summary

Sweetwater Care is seeking a dedicated Speech-Language Pathologist to assist patients with speech, language, swallowing and cognitive disorders in a skilled nursing facility. You will evaluate patients, develop and implement individualized treatment plans, and coordinate care with physicians and therapists to maximize communication and swallowing safety.

The role involves administering standardized assessments, monitoring progress, and documenting sessions in patient records.

Qualifications

• Experience in SNF settings.
• Knowledge of Medicare/Medicaid reimbursement.
• Ability to perform assessments and document progress.

Responsibilities

• Direct all aspects of quality patient care including evaluation, treatment planning and documentation.
• Provide professional speech-language treatment for rehabilitation of speech, language, swallowing and cognitive disorders.
• Develop treatment plans with short-/long-term goals and obtain physician approvals.
• Collaborate with other therapists and healthcare professionals to coordinate patient care.
• Document evaluations, progress notes, and discharge summaries in a timely manner.
• Ensure delivery of quality rehab services and maintain compliance with policies.
