Job summary

South Creek Post Acute is seeking a compassionate Speech Therapist to evaluate residents and deliver evidence-based therapy. You will assess speech, language, cognitive, communication, and swallowing disorders and create individualized plans to improve daily functioning.

The role requires an active Washington license; a Master’s in Speech-Language Pathology is expected, with CCC-SLP preferred. Join a team dedicated to resident-centered care and collaborative practice in a supportive setting.

Qualifications

  • Licensed Speech-Language Pathologist in Washington State.
  • Master's degree in Speech-Language Pathology from an accredited program.
  • CCC-SLP certification preferred.

Responsibilities

  • Evaluate residents for speech, language, cognitive, communication, and swallowing disorders.
  • Develop and implement individualized treatment plans to improve functional outcomes.
  • Provide skilled therapy services per physician orders and care plans.
  • Collaborate with physicians, nursing staff, families, and the interdisciplinary team.
  • Maintain accurate documentation and comply with regulatory requirements.
  • Educate residents, families, and caregivers on treatment strategies and progress.
