Job summary

Lacamas Creek Post Acute is seeking a Speech Language Pathologist to join our rehabilitation team. You will evaluate residents, create individualized treatment plans, and provide therapy across speech, language, cognition, and swallowing domains.

Work closely with nursing and interdisciplinary teams, educate residents and families about goals and care plans, and ensure timely, compliant documentation in a supportive environment.

Qualifications

  • Current Washington Speech Language Pathologist license.
  • CCC-SLP certification preferred but not required.
  • Strong communication and organizational skills.
  • Experience in skilled nursing or post-acute care preferred.

Responsibilities

  • Evaluate residents and develop individualized speech therapy treatment plans.
  • Provide therapy for speech, language, cognition, communication and swallowing.
  • Collaborate with nursing and therapy teams to support patient progress.
  • Educate residents and families on goals and care plans, document accurately.
