Job summary

Creekside Village is seeking a Per Diem Speech-Language Pathologist to provide rehabilitative therapy for communication, cognitive and swallowing disorders in a long-term care setting.

The SLP must be a graduate of an accredited program, ASHA CCCs, and hold a current PA state license; one year in LTC or related settings is preferred. The role reports to the Director of Rehabilitation Services and emphasizes objective documentation and compliance with clinical standards.

Qualifications

• Must have completed an accredited Speech-Language Pathology program.
• ASHA CCC certification is required.
• Current and active Pennsylvania state license required.
• One year of long-term care or related setting experience preferred.

Responsibilities

• Provide rehabilitative therapy to patients with communication, cognitive and swallowing disorders.
• Document clinical progress with objective measures and justification for continued services.
• Comply with regulatory and clinical practice requirements; maintain accurate documentation.
• Report to the Director of Rehabilitation Services.

The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

Job Requirements

• Must be a graduate of an accredited program in Speech-Language Pathology
• Must be ASHA certified (CCCs)
• Must have current and active license with the state of Pennsylvania
• One year experience in long-term care or related setting preferred
