Job summary

Creekside Village is seeking a Part Time Speech Language Pathologist to provide rehabilitative therapy for patients with communication, cognitive and swallowing disorders. The role involves improving function and returning patients to their prior level of functioning, with documentation that supports continued care.

The SLP will follow all regulatory and clinical practice requirements and report to the Director of Rehabilitation Services.

Qualifications

• Graduate of accredited Speech-Language Pathology program.
• ASHA CCCs certification required.
• Active state license in Pennsylvania.
• One year experience in long-term care or related setting preferred.

Responsibilities

• Provide rehabilitative therapy to patients with communication, cognitive and swallowing disorders.
• Document progress with objective measures and justify continued or discontinued services.
• Comply with all regulatory and clinical practice requirements.

The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

Job Requirements

• Must be a graduate of an accredited program in Speech‑Language Pathology
• Must be ASHA certified (CCCs)
• Must have current and active license with the state of Pennsylvania
• One year experience in long-term care or related setting preferred
