Job summary

Creekside Village is seeking a Full Time Speech-Language Pathologist to provide rehabilitative therapy for communication, cognitive and swallowing disorders to residents in a long-term care setting.

The SLP will document progress with objective measures and coordinate with the Director of Rehabilitation Services to ensure quality care and appropriate discharge planning. Licensure and ASHA CCCs are required, with PA licensure current and active.

Qualifications

• Must be a graduate of an accredited program in Speech-Language Pathology.
• Must be ASHA certified (CCC)
• Must have current and active license with the state of Pennsylvania.

Responsibilities

• Provide rehabilitative therapy to patients with communication, cognitive and swallowing disorders.
• Document clinical progress with objective measures and justify continued services.
• Report to the Director of Rehabilitation Services and coordinate with the care team.
