Job summary

Creekside Village is seeking a Per Diem Speech-Language Pathologist to provide rehabilitative therapy for communication, cognitive and swallowing disorders. The clinician will assess, treat, and document progress for patients referred for rehab, ensuring care meets regulatory and clinical practice standards.

This role reports to the Director of Rehabilitation Services and emphasizes objective measures, periodic updates, and aiming for improved function and return to prior levels of independence.

Qualifications

• Must be graduate of an accredited program in Speech-Language Pathology.
• Must be ASHA certified (CCC) or CCCs.
• Must have current and active license with the state of Pennsylvania.
• One year experience in long-term care or related setting preferred.

Responsibilities

• Provide rehabilitative therapy for communication, cognitive and swallowing disorders.
• Document therapy sessions with objective measures and justification for continued or discontinued services.
• Follow regulatory and clinical practice requirements in delivering therapy.
• Report to the Director of Rehabilitation Services.
