Job summary

21st Century Home Health Services, the Bay Area’s largest home health agency, seeks a Speech Therapist/Language Pathologist to visit patients in their homes across the SF Bay Area. You will work with an interdisciplinary team, ensuring high-quality care and regulatory compliance.

Requirements include a CA Speech Therapist License, BLS/CPR, and a CA Driver's License. Competitive compensation, excellent benefits, and mileage reimbursement are offered.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and human behavior related to illness/injury.
• Experience in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team.

Responsibilities

• Visit patients in their homes as part of care delivery.
• Collaborate with an interdisciplinary team to coordinate patient care.
• Participate in case management and regulatory compliance.
• Navigate electronic charting and company web tools.

To be a success as Speech Therapist/Language Pathologist (ST)

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
