Job summary

21st Century Home Health Services (21HHS) seeks a Speech Therapist/Language Pathologist to provide in-home evaluations and therapies. You will work with an interdisciplinary team to evaluate, treat, and educate patients with speech, language, cognitive, and fluency disorders.

You will serve as a patient advocate, participate in care coordination, and contribute to high-quality outcomes while supporting a flexible schedule and generous benefits.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Prior experience in home health care is preferred.
• Knowledge of regulatory guidelines for home care is a plus.

Responsibilities

• Provide skilled speech, language, cognitive and fluency services to patients at home.
• Evaluate and treat communication disorders in partnership with the interdisciplinary care team.
• Educate patients and act as patient advocate within the care team.
• Collaborate with Case Manager to coordinate care and discharge planning.

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST)

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

To be a success as Speech Therapist/Language Pathologist (ST)

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired

$110,000 - $125,000 a year

Anticipated Wage range Speech Therapist: 110,000.00-125,000.00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match)
