Job summary

Primrose Post Acute Kumquat Holdings, LLC is seeking a Staff Speech-Language Pathologist to evaluate and treat residents, develop personalized treatment plans, and document services in medical records. The role includes training facility staff and supervising CFY/ students as needed.

You will work with physicians, nursing and therapy teams, participate in care and rehabilitation meetings, ensure compliance, and educate residents and families on goals and home exercise programs.

Qualifications

• Bachelor's degree in Speech-Language Pathology; Master's preferred.
• Licensed as a Speech Therapist in the state; ongoing CE requirements.
• Proficient with medical documentation and health records software.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop and implement individualized treatment plans to improve communication and swallowing.
• Supervise CFY Speech Clinical Fellows or students as applicable.
• Collaborate with physicians, nursing and multidisciplinary teams; participate in care conferences.
