Job summary

Willoughby Post Acute is seeking a licensed Speech-Language Pathologist to evaluate and treat residents, develop individualized treatment plans, and collaborate with families and the care team in Willoughby, Ohio.

The role includes supervising CFY speech fellows and students, providing staff in-services, ensuring timely documentation, and supporting quality improvement initiatives within the facility.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; master’s or doctoral degree preferred.
• Proficient with Net Health and PointClickCare.
• Ability to read technical procedures, policies, and manuals.
• Strong math and problem-solving skills.
• Licensed Speech Therapist in the state; CPR certification if required.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve communication and swallowing.
• Supervise CFY Speech Clinical Fellows and students.
• Communicate progress and discharge planning with physicians, nursing staff, residents and families.
• Participate in Resident Care conferences, Utilization Review and Rehabilitation meetings as needed.
• Provide staff in-services and training programs.
• Complete timely, accurate, and compliant documentation per requirements.
• Maintain a safe treatment environment and infection control protocols.
• Educate residents, families, and caregivers on goals and home programs.
• Recommend and facilitate ordering of durable medical equipment as needed.
• Contribute to quality assurance and rehabilitation program development.
