Concept Rehab, Inc. is seeking a dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in a skilled nursing and post-acute care setting in Willoughby, OH.

You will evaluate and treat adults and geriatric patients with communication, cognitive, and swallowing challenges to improve daily function and quality of life. Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Ohio Speech-Language Pathologist license.
• CCC-SLP preferred.
• Clinical Fellowship (CFY) candidates encouraged to apply.
• Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred.
• Strong communication, problem-solving, and clinical decision-making skills.
• Passion for improving patient outcomes and quality of life.

Responsabilités

• Perform comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on patient needs and functional goals.
• Deliver skilled interventions for communication, cognitive-linguistic, voice, and dysphagia disorders.
• Assess swallowing safety and recommend dietary modifications when indicated.
• Collaborate with nursing, physicians, dietitians, and rehabilitation professionals.
• Educate patients and families regarding treatment strategies and recommendations.
• Monitor progress and modify treatment to maximize outcomes.
• Participate in interdisciplinary care planning, quality initiatives, and discharge planning.
• Maintain accurate, timely, and compliant documentation.
• Support evidence-based clinical practice and patient-centered care.
