Job summary

Capital Health is seeking a Speech Pathologist (Diem) IP to assess and treat patients in hospital, outpatient clinic, and home-based programs. You will develop treatment plans, perform evaluations, and document care accurately.

The role requires state licensure as a Speech-Language Pathologist in New Jersey, BLS certification, and at least one year of relevant experience with acute or pediatric populations.

Qualifications

• Education: Graduation from accredited speech language pathology school or education approved by NJ State Board of Speech Language Pathology Examiners.
• Licensed as a speech-language pathologist by the New Jersey Division of Consumer Affairs, Audiology and Speech-Language Pathology Advisory Committee.
• AHA BLS - Healthcare Provider.
• Experience: One year experience preferred with patient populations including acute adult inpatient, adult outpatient, or pediatric.
• Stroke-related continuing education (2 hours annually for acute care).

Responsibilities

• Evaluate and treat patients in hospital, outpatient clinic and home under the hospital at home program.
• Develop treatment plans and establish measurable goals; implement procedures and exercises.
• Instruct patients, families, and caregivers to maximize function and discharge readiness.
• Coordinate care with physicians, nurses, case managers and CH team; communicate and advocate for patients.
• Document evaluation, treatment, attendance and discharge clearly and timely; ensure billing accuracy.
