Life Care Centers of America is seeking a Speech Language Pathologist to provide direct rehab care in speech therapy to patients upon physician referral in a skilled nursing facility setting. The role requires a graduate degree in speech-language pathology and ASHA credentialing or eligible CFY, with current state licensure.

You will work with an interdisciplinary team to support long-term and short-term rehab goals, document progress, and participate in community health initiatives.

Qualifications

• Graduate from an accredited speech therapy program (MS/MA).
• ASHA CCCs or eligibility for CFY; must earn/maintain ASHA credentials.
• Current state licensure and CPR certification obtainable.

Responsibilities

• Establish and adjust functional therapy goals for patients.
• Oversee and evaluate CFYs when applicable (SLP CCC).
• Follow treatment plans under supervision and document progress.
• Chart patient data timely and accurately using therapy software.
• Collaborate with an interdisciplinary team to support patient care.
