Job summary

Bridge Home Health & Hospice is seeking a Speech-Language Pathologist to implement standards of care for SLP services within the Sonoma territory. The role involves home-based evaluations, care planning, and delivering speech therapy with specialized techniques to improve patient function.

Qualified candidates have an SLP degree and CA licensure or eligibility, 1+ year of clinical experience, and strong communication skills. CPR certification and coordination with physicians are required.

Qualifications

• Master’s or doctoral degree in speech-language pathology (SLP).
• Licensed as a speech-language pathologist by the State or eligible for licensure.
• Minimum 1 year of clinical SLP experience.
• Strong verbal and written communication and organizational skills.
• CPR certification.

Responsibilities

• Conduct evaluations and assess home environment for SLP needs.
• Identify functional speech defects and plan care to improve function.
• Administer speech therapy using specialized techniques/equipment.
• Recommend communication devices as indicated.
• Coordinate with referring physicians and care teams.
• Participate in case conferences and in-services as required.
• Prepare and submit notes per organizational policy.
• Maintain clinical competency in SLP practice.
• Refer to other services as appropriate.
• May supervise home health aides in related tasks.
• Attend other duties to facilitate delivery of SLP services.
