Job summary

Silverwood Post Acute seeks a licensed Speech-Language Pathologist to evaluate patients within 24 hours of referral and to develop and obtain approval for treatment plans from the referring physician. The role includes supervising Speech Language Pathologist Assistants, documenting progress, and contributing to discharge planning.

The position requires a state-licensed SLP, with a preference for those holding a Master’s or Doctorate in Speech-Language Pathology and ongoing continuing education.

Qualifications

• Bachelor's degree in Speech-Language Pathology required; licensure in the state of practice.
• Masters or Doctorate in Speech-Language Pathology preferred.
• Must maintain licensure and meet continuing education requirements.
• Ability to supervise Speech Language Pathologist Assistants and collaborate with health team.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per physician plan and document progress in medical records.
• Supervise Speech Language Pathologist Assistants in direct patient care.
• Record evaluations and daily/weekly progress notes and discharge documentation.
• Participate in care conferences and in-service trainings for staff.
• Instruct families and staff for maintenance and discharge planning.
• Act as clinical preceptor for CFY/RFP students and supervisees.
