Job summary

Pikes Peak Post Acute in Oregon is seeking a licensed Speech-Language Pathologist to evaluate and treat residents according to physician plans. You will develop treatment plans, obtain physician approvals, and provide care within the established therapy program.

You will supervise Speech Language Pathologist Assistants, document progress, coordinate with the health team, and participate in discharge planning and staff training.

Qualifications

• Bachelor's degree in Speech-Language Pathology required.
• Licensed as a Speech Therapist in the state.
• Must meet continuing education requirements per state.
• License in good standing with the state board.

Responsibilities

• Evaluate patients within 24 hours, or 1 business day of physician referral.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Supervise Speech Language Pathologist Assistants in direct patient care and related activities.
• Record evaluations, daily treatment notes, weekly progress notes, per policy and payer requirements.
• Instruct patient’s families or nursing staff in maintenance and caregiver training for discharge.
