Job summary

DayToDay Health seeks a Speech Language Pathologist to provide one-on-one therapy to older adults in private homes or senior living communities. You will perform evaluations, develop care plans, and deliver evidence-based treatments while documenting in EMR.

You will collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate. In-home care demands flexibility, autonomy, and a commitment to improving patient outcomes.

Qualifications

• Valid state license or eligibility to practice speech-language pathology
• Accredited degree in speech-language pathology
• EMR proficiency
• CPR certification
• ASHA CCC certification
• Physical ability to perform in-home care activities, including lifting, repositioning, and driving
