Job summary

Proforma Health is seeking a Speech-Language Pathologist to provide one-on-one therapy for older adults in private homes and senior living communities.

Responsibilities include evaluations, creating personalized treatment plans, evidence-based therapy, and collaboration with an interdisciplinary team. Telehealth may be used to extend access to care and support patients in aging in place.

Qualifications

• Must hold or be eligible for state licensure in Speech-Language Pathology.
• Degree from an accredited SLP program.
• CPR certification required.

• Utilize telehealth and virtual care to enhance service delivery.

Physical & Other Requirements

• Ability to perform moderate physical effort, including lifting and repositioning patients
• Capability to drive to patient locations and wear PPE as needed
