Job summary

Proforma Health is seeking a Licensed Speech Language Pathologist to provide personalized care to older adults in private homes and senior living communities.

Responsibilities include evaluations, treatment planning and therapy delivery to support patient wellness, mobility, and independence; documentation in EMR, interdisciplinary collaboration, and caregiver education with telehealth as needed.

Qualifications

• Valid SLP license required.
• ASHA CCC credential preferred.
• Degree from an accredited Speech-Language Pathology program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Job description

Role:

Physical Requirements:

Ability to perform moderate physical tasks including lifting, repositioning patients, standing, walking, climbing stairs, and driving. PPE use required when necessary.
