Qualificações

• Graduate of ACPE-accredited School of Pharmacy with a B.S. Pharmacy or PharmD.
• Current, unrestricted pharmacist license in state of residence; ongoing education credits to maintain licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.

Responsabilidades

• Provide telephonic patient education for specialty pharmacy patients focusing on disease state management, medication administration, storage, and program adherence.
• Document clinical consultations and interventions in the pharmacy software system per internal policies and external regulatory bodies.
• Interpret and evaluate prescriptions; contact providers/patients to verify details to expedite processing; verify controlled substance validity.
• Coordinate with physicians for prescriptions, discuss pharmaceutical questions with the care team, and support transfers to optimize therapy and generic/biosimilar use when possible.
• Ensure compliance with regulations and company policies; stay current with new therapies and guidelines.
• Back up other pharmacists and train less experienced staff; participate in quality and safety initiatives.
