Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic patient consultations about specialty medications and disease-state management. You will reconcile medications, screen for interactions, and provide side-effect management recommendations in a patient-centered care model.

You will collaborate with prescribers, document interventions in our pharmacy software, and support adherence, quality standards, and production metrics while staying current with specialty therapies.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Current, unrestricted pharmacist license in state of residence; must continuously obtain education credits to maintain licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.
• Strong clinical background required.
• Experience with Specialty Pharmacy medications desirable.
• Participate in, adhere to, and support compliance program objectives.
• Ability to consistently interact cooperatively and respectfully with other employees.

Responsibilities

• Provide telephonic patient education for specialty pharmacy patients, focusing on disease-state management, side effect mitigation, administration, storage, and therapeutic appropriateness.
• Document clinical consultations and interventions in the pharmacy software system per policies and regulatory requirements.
• Interpret and evaluate prescriptions, contact providers/patients to verify details to expedite processing, and confirm controlled substance validity.
• Contact physicians for new prescriptions and coordinate with prescribers for transfers or generic/biosimilar alternatives.
• Verify prescription data entered into the system and answer clinical questions from the pharmacy team.
• Adapt to new workflows, stay current with specialty therapies and new drug approvals.
• Back up pharmacists and train less experienced staff as needed.
• Document and report medication errors per ISMP standards and company policies.
• Work in accordance with all applicable laws and high ethical standards.
