Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic consultations with patients regarding specialty medications and disease state management. The role includes reconciliation, drug interaction screening, and side effect management, with emphasis on patient-centered care and meeting production metrics.

Qualifications

• Graduate of an ACPE accredited School of Pharmacy with a PharmD or B.S. in Pharmacy.
• Current, unrestricted pharmacist license in state of residence; ongoing education credits.
• At least two years of experience as a licensed Pharmacist preferred.
• Strong clinical background; specialty pharmacy experience desirable.

Responsibilities

• Provide telephonic patient education for specialty pharmacy patients.
• Document consultations and interventions in pharmacy software per policy and URAC/regs.
• Interpret prescriptions and verify drug strength, directions, and diagnosis with providers.
• Contact physicians for prescriptions, clarify information to expedite orders, and convert to generics when possible.
• Verify and correct prescription info in the system; respond to clinical questions from the pharmacy team.
• Stay current with specialty pharmacy therapies and new drug approvals.
