Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. This role conducts telephonic consultations with patients on specialty medications, performs medication reconciliation, screens for drug interactions, and provides side effect management guidance.

The pharmacist will interpret prescriptions, coordinate with prescribers, and help expedite order processing. The position emphasizes independence, patient-centered care, and meeting department production metrics while upholding

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy.
• Current, unrestricted pharmacist license in state of residence; must continually obtain education credits to maintain active licensure.
• At least two years of experience as a practicing licensed Pharmacist preferred.
• Experience and/or strong understanding of Specialty Pharmacy medications desirable.

Responsibilities

• Provide patient education through telephonic consultations for patients of the specialty pharmacy in accordance with Lumicera's Patient Management Program.
• Provide detailed clinical documentation of patient consultations and interventions in pharmacy software system in compliance with internal policies and external regulatory bodies.
• Interpret and clinically evaluate prescriptions for specialty medications; contact providers/patients to verify information to expedite processing.
• Contact physicians for new prescriptions and collaborate with physicians and nurses on pharmaceutical questions; assist with prescription transfers and generic conversions.
• Verify prescription information entered in the system and answer clinical questions from the pharmacy team.
• Stay current with advancements in specialty therapies and guidelines; adapt to new workflows and processes.
• Back up other pharmacists and train less experienced staff.
