Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic patient consultations regarding specialty medications and disease management. You will conduct medication reconciliation, screen for interactions, and provide side effect management with a patient-centered approach.

Responsibilities include documenting interventions, clarifying prescriptions with providers, expediting orders, and recommending substitutions when appropriate.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy (B.S. Pharmacy or PharmD).
• Current, unrestricted pharmacist license in state of residence.
• At least two years of licensed pharmacist experience.
• Strong clinical background; specialty pharmacy experience desirable.
• Participate in and support compliance program objectives.
• The ability to interact cooperatively with team members.

Responsabilités

• Provide telephonic consultations with patients regarding specialty medications and disease management.
• Document patient interactions and interventions in pharmacy software per policies.
• Interpret prescriptions, verify details with prescribers, and expedite orders.
• Advise on generic or biosimilar substitutions when appropriate.
• Stay current with advances in specialty therapies and guidelines.
• Back up other pharmacists and train junior staff as needed.
• Ensure compliance with federal/state laws and ethical standards.
• Investigate and report medication errors per procedures.
