Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic patient consultations on specialty medications and disease state management. The role includes medication reconciliation, drug interaction screening, and advising on side effects to enhance patient care.

The pharmacist will interpret prescriptions, contact prescribers for clarification, and expedite orders while meeting production metrics and service levels.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.
• Current, unrestricted pharmacist license in state of residence.
• Two years of experience as a practicing licensed Pharmacist preferred.
• Strong clinical background.
• Experience with Specialty Pharmacy medications desirable.

Responsibilities

• Provide telephonic patient education and consultations for specialty medications.
• Document clinical consultations in pharmacy software per policy and regulatory bodies.
• Evaluate prescriptions for drug strength and directions; verify information with providers or patients.
• Coordinate with physicians for prescription clarification or transfers; promote generics when possible.
• Adhere to legal and ethical standards and support quality improvement initiatives.
• Back up other pharmacists and train less experienced staff.
• Identify and report medication errors per ISMP standards.
