Job summary

Community Bridges, Inc. (CBI) is seeking a Specialty Case Manager to provide community-based case management for individuals with serious mental illness and/or high-risk needs. You will plan, coordinate, advocate, and monitor services to help patients access health care, housing, employment, education, and social supports.

The role emphasizes recovery, safety, and timely documentation, with a focus on outreach, collaboration with treatment teams, and adherence to agency policies.

Qualifications

• High school diploma or GED required.
• Associate’s degree or higher in a field related to behavioral health preferred.
• One year of experience in a behavioral health position providing outreach and engagement activities.
• Current Arizona driver’s license.
• Arizona fingerprint clearance card.
• Jail clearance if applicable.
• 39‑month MVR.

Responsibilities

• Provide community-based case management to patients with serious mental illness and/or high risk.
• Assist in planning, connecting, advocacy, coordination, and monitoring to access health services, housing, employment, education, and social services.
• Document services accurately and timely.
• Promote recovery and a safe recovery environment for clients, implementing advanced techniques for high‑risk individuals.
