Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide community and office-based case management, coordinating and monitoring patient needs and progress. You will help clients access health services, financial aid, employment, education, and social supports while promoting recovery in a supportive environment.

The role emphasizes planning, documentation, and collaboration with care teams to ensure individualized treatment plans are implemented with accuracy and timeliness.

Qualifications

• Highschool diploma or GED required. Associates degree of higher in a field related to behavioral health preferred.
• 1-3 years of experience working with the GMH/SU population required.
• 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
• 1-3 years of experience in a medical setting preferred.
• Minimum of 6 months of recovery from substance use and/or mental health disorders preferred.
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
• Valid Identification required.
• Current AZ Driver’s License (valid and in good standing) preferred.
• Clean 39-month driving record preferred
• Arizona Fingerprint Clearance Card.

Responsibilities

• Provide case management services in either the community and/or in office setting.
• Assist the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress.
• Assist patients in gaining access to health services, financial assistance, employment, education, social services and natural supports.
• Assess patient needs, develop and monitor individualized treatment plans and documents services provided in an accurate and timely manner.
• Embraces the recovery philosophy and promotes a safe recovery environment for clients.
