Community Bridges, Inc. (CBI) seeks a Rehabilitation Specialist to work with SMI clients, coordinating services with Case Managers and Clinical Coordinators.

This role emphasizes field mobility, with 80% of time spent in the community assisting clients and reviewing progress. The position requires a high school diploma or GED, with preferred associates in behavioral health, and relevant outreach experience.

Qualifications

• High school diploma or GED required; associate degree or higher in behavioral health preferred.
• 1 year experience with the SMI population across multiple settings and levels of care preferred.
• 1 year in a behavioral health role involving outreach and engagement preferred.
• Minimum of 6 months recovery from substance use and/or mental health disorders preferred.
• Current AZ Driver’s License in good standing.
• Clear 39-month Motor Vehicle Record (MVR) required.
• Fingerprint Clearance Card required.
• Behavioral Health Technician (BHT) certification per ACA and CBI policies.

Responsibilities

• Assist SMI clients in obtaining or maintaining supports for daily functioning.
• Collaborate with the client's Case Manager and Clinical Coordinator to ensure services are delivered.
• Maintain mobility with 80% field time visiting clients to review progress and arrange services.
