Job summary

AdvantageCare Physicians (ACPNY) is seeking an RN to provide professional nursing care and serve as a liaison among patients, staff and physicians within its extensive New York network. The role emphasizes patient safety, care coordination, and education while aligning with the Advanced Care Model.

The nurse will supervise LPNs and CTA IIs on the specialty team, participate in staffing and training, and support daily huddles and care plans with physicians and practice administrators.

Qualifications

• Graduate of an accredited school of nursing
• Current unencumbered New York State Registered Nurse license
• BLS
• ACLS required for GI and Cardiology

Responsibilities

• Provide general nursing care to patients including administering prescribed medications and treatments
• Coordinate and manage daily huddles and prepare materials
• Review patient charts and active orders
• Administer medications as directed by provider
• Provide patient education on self-management of chronic conditions
