Job summary

AdvantageCare Physicians (ACPNY) is a growing primary and specialty care practice serving half a million patients in the New York metropolitan area. This role provides professional nursing care, coordinates care with physicians, and supervises LPNs and CTA IIs to maintain high standards of patient safety and satisfaction.

The Nurse will function as a clinical liaison, manage daily huddles, review patient charts, and facilitate care plans and patient education.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor's degree in nursing preferred.
• Current unencumbered New York State Registered Nurse license.
• BLS
• ACLS required for GI and Cardiology.

Responsibilities

• Provide professional nursing care to patients and proactively support efforts to ensure patient satisfaction, safety, and quality of care.
• Serve as primary liaison between patients, staff and physicians and support the Advanced Care Model.
• Organize and provide clinical supervision to the specialty patient care team including delegation, mentoring, evaluating, educating, scheduling and counseling.
