Job summary

NLACRC is seeking a Consumer Services Specialist – Specialized Service in Chatsworth, CA. The role provides information, advocacy, and case management for individuals with complex needs, coordinating services with families and providers to build comprehensive service plans.

A strong regulatory understanding and collaboration with interdisciplinary teams are essential. Requires a Bachelor’s degree and four years of related experience; master’s degree may substitute for part of the experience.

Qualifications

• Bachelor's degree plus four years of related experience required.
• Master's degree may substitute for two years of experience.
• Experience with complex needs and disability services preferred.

Responsibilities

• Provide information, advocacy, and case management stabilization for consumers with complex needs.
• Coordinate case management services to stabilize for up to 12 months on average 1:25 caseload.
• Develop and implement comprehensive service plans with families and providers.
• Facilitate communication among consumers, families, guardians, outside agencies, and staff.
