Job summary

North Los Angeles County Regional Center (NLACRC) in Chatsworth, CA is seeking a Consumer Services Specialist – Specialized Service. The role provides information, advocacy, and case management for individuals with complex needs identified in program language, coordinating services with families and providers.

You will work to develop and monitor service plans, communicate with diverse stakeholders, and maintain documentation.

Qualifications

• Bachelor’s degree and four years of related experience.
• Master’s degree may substitute for two years of experience.
• Experience servicing complex needs preferred; regional center or related agency experience preferred.

Responsibilities

• Provides information, advocacy, and case management for consumers with complex needs.
• Develops and implements comprehensive service plans with families, clinicians, and service providers.
• Facilitates communication among consumers, families, guardians, agencies, and stakeholders.
• Maintains case records, funding and billing records; produces reports.
• Participates in committees and meets regularly with individuals in their homes or community.
