Job summary

Optima Medical Management Group is seeking a Lead Care Manager (LCM) to design personalized care plans, coordinate member services, and collaborate with families and providers to ensure timely access to care.

Required: high school diploma or GED and Spanish bilingual capability; 1+ year in case management or member care; valid CA driver's license. This role involves travel to Stockton, CA and in-office visits with telehealth support.

Qualifications

• High school diploma or GED required.
• MA certificate or medical terminology knowledge preferred.

Responsibilities

• Interview members to assess medical and social determinants; develop individualized care plans.
• Respond to member inquiries and concerns; ensure compliance with hospital and legal requirements.
• Collaborate with interdisciplinary teams; locate medical and social resources; coordinate care plans.
• Maintain ongoing contact with members via telehealth and in-person visits.
• Advocate for members; coordinate appointments and treatment plans; evaluate progress.
• Maintain empathy and professionalism when contacting members and families.
