Optima Medical Management Group in California seeks a Lead Care Manager to design personalized care plans, coordinate services, and work closely with members, families, and primary care providers to ensure timely access to care.

You will provide emotional support, resolve administrative issues, and help members become self-sufficient in health while participating in the Enhanced Care Management program. Travel is required for outreach.

Mansioni

• Interview members to assess medical and social determinants and develop individualized care plans.

• Collaborate with interdisciplinary teams and coordinate social service plans.
• Maintain ongoing contact with members via telehealth and in-person visits.
• Advocate for members; coordinate appointments and treatment plans; monitor progress.
