Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management, member care, call center, or related.
• Valid California driver’s license and vehicle insurance required.
• MA certificate or medical terminology knowledge preferred.

Responsibilities

• Interview members to assess medical and social determinants of health and develop care plans.
• Respond to member inquiries, ensuring compliance with hospital and legal requirements.
• Coordinate with interdisciplinary teams, locate resources, and arrange social service plans.
• Maintain ongoing contact with members via telehealth and in-person visits.
• Advocate for members, coordinate with providers, and monitor progress.
