Qualifications

• 1 year intake experience preferred.

Responsibilities

• Receive referrals from referral sources and process referrals from hospitals, nursing homes, and physicians.
• Verify patient insurance to determine eligibility.
• Educate patients on services provided by the agency.
• Determine what services were approved by insurance.
• Contact patients to educate them on available Home Care services and assist with skilled and non-skilled needs.
