Job summary

Serve the People, Inc. is seeking a Referral Coordinator to coordinate patient referrals, follow up with labs and specialty providers, and communicate with care teams to ensure timely care and documentation. Qualifications

• High school diploma or equivalent.
• 1 year of experience in customer service/referral coordinator.
• Experience with common health care programs, preferred.
• Experience working with underserved and diverse populations, preferred.
• Basic knowledge of medical terminology, preferred.
• Electronic Health Record (EHR) experience, preferred.

Responsibilities

• Coordinate patient referrals and follow up with labs, diagnostics, and provider notes.
• Follow up with referral providers to obtain consult notes and confirm test results.
• Communicate with care teams about patient needs and scheduling.
• Notify patients of referral status updates and ensure proper closure in the system.
• Maintain confidentiality of medical records and patient information according to HIPAA.
