Job summary

Fair Haven Community Health Care is seeking a Referrals Navigator to support patients and clinical teams by guiding referrals from initiation through prior authorization, scheduling, and follow-up. You will document all steps in the patient’s Electronic Health Record and communicate with referring clinicians to ensure timely care.

Ideal candidates have a year of healthcare experience and bilingual English/Spanish skills.

Qualifications

• One year of experience in a health care setting required.
• Referrals experience preferred.
• Proficiency in data entry using a database such as Excel and/or Access.

Responsibilities

• Manages all aspects of external referrals in partnership with clinical teams.
• Obtains prior authorization for referrals when necessary.
• Schedules appointments and coordinates with referral sites.
• Documents referrals in the patient’s EHR and communicates with referring clinicians.
• Educates patients on referral requirements, sites, and appointment details.
• Updates clinical leadership on changes to the referrals process.
