Job summary

Salud Para La Gente, Inc. is seeking a Referrals Specialist to supervise the Salud-wide specialty referral program across all sites. You will process referrals, verify eligibility, and ensure accurate registration in the EHR.

The role requires coordinating with patients, families, staff, and providers to deliver timely and precise referral information. Responsibilities include ensuring 24-hour urgent referrals, 5-business-day routine referrals, and ongoing tracking of all referrals, while

Qualifications

• Must interact effectively with patients and maintain a pleasant demeanor with callers.
• Bilingual English/Spanish is required.
• Basic computer knowledge and experience with electronic health records.

Responsibilities

• Follow Medical Provider orders and complete referrals promptly.
• Coordinate STAT/URGENT referrals within 24 hours.
• Process Routine referrals within 5 business days.
• Monitor referral queue and close open orders in scheduled reviews (30/60/90 days).
• Notify patients of appointments and assist with scheduling, interpretation and transportation services.
