Job summary

HCA Houston Healthcare Southeast seeks a Social Worker MSW PRN to perform non-clinical, biopsychosocial assessments and interventions on inpatient floors and the outpatient emergency department. The MSW delivers crisis intervention, patient/family support, discharge planning, and resource referrals in a patient- and family-centered care model.

The role requires collaboration with RN case managers, physicians, and insurers to address barriers to discharge and recovery, while maintaining knowledge

Qualifications

• Master's degree required.
• MSW licensure or intern status required (LCSW/LMSW/RCSW-I).

Responsibilities

• Collaborate with care team to identify and address patient needs.
• Perform biopsychosocial assessments and discharge planning for high-risk patients.
• Provide crisis intervention and emotional support to patients and families.
• Coordinate meetings among care team, patients, and families.
• Assist in end-of-life conversations and resource referrals.
