Qualifications

• Masters in social work from an accredited college.
• Texas LMSW license or equivalent.
• Hospital setting experience preferred.

Responsibilities

• Perform biopsychosocial assessments for inpatients and ED.
• Provide crisis intervention and emotional support.
• Develop discharge plans and coordinate post-acute care.
• Collaborate with care team to address barriers to discharge.
• Assist with referrals for financial assistance and resources.
• Engage families in decision making and advocacy.
