Job summary

Methodist Hospital Northeast, part of HCA Healthcare, is seeking a Social Worker (MSW) to perform biopsychosocial assessments and interventions for inpatients and the outpatient ED. You will collaborate with care teams, address discharge barriers, and provide crisis support and resource referrals to patients and families.

The role requires an MSW, LMSW licensure, and experience in a hospital setting; bilingual skills are preferred.

Qualifications

• Requires a Master’s Degree in Social Work from an accredited college or university.
• Two years experience in hospital setting preferred.
• Social Work license (LMSW)

Responsibilities

• Collaborate with Rn case managers, acute care nurses, physicians, and insurers to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes; understand managed-care concepts and length-of-stay management.
• Perform detailed assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team and patients/families; support patient preferences and values during decisions.
• Intervene for psychosocial needs and provide counseling, advocacy, and referrals to meet emotional, social, and financial care aspects.
• Provide emotional support during complex or emotionally charged issues (end-of-life, death notifications, crisis interventions).
