Qualificações

• Master's degree in social work from an accredited college or university.

Responsabilidades

• Collaborate with RNs, case managers, physicians, and payers to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare/Medicaid and private payer regulations and length-of-stay concepts.
• Perform assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families, and caregivers; support decision making.
• Provide counseling and resources for emotional, social, and financial aspects of care.
