Job summary

HCA Houston Healthcare Southeast is seeking a Social Worker MSW PRN to perform non-clinical biopsychosocial assessments and interventions on inpatient floors and in the outpatient ED.

The role includes crisis intervention, discharge planning for high-risk patients, and collaboration with care teams to address barriers to discharge and post-acute needs, with a focus on patient-centered care and family engagement.

Qualifications

• Masters Degree required.
• (LCSW) Clinical Social Worker, or (LMSW/RCSW-I) Lic Master Social Worker/Reg Clin SW-Intern is preferred.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, and insurers to identify and address patient needs.
• Identify and address barriers to discharge and recovery across financial, emotional, psychosocial, medical, and spiritual domains.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts related to length of stay and discharge planning.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care teams, patients, families, and caregivers; support patient-centered decision-making.
• Provide psychosocial support, counseling, and referrals to meet emotional, social, and financial aspects of care.
