Job summary

Tri-Counties Regional Center seeks a Service Coordinator 2 to provide person-centered consultation, advocacy, and service coordination for individuals with developmental disabilities in Santa Barbara County.

The role includes planning, information and referral, and collaboration with schools, day programs, and community agencies. Strong communication and organizational skills are required, with travel across locations as needed.

Qualifications

• Master’s or Bachelor’s degree in human services field with 2 years of applicable experience.
• 2 years experience in case management within a related field.
• 1 year experience with individuals with developmental disabilities preferred.

Responsibilities

• Maintain regular contact with individuals served and develop IFSPs and IPPs.
• Participate in IDT meetings and community meetings as needed.
• Provide information and referral to community services and resources.
• Coordinate and secure funded services including behavior services and respite.
• Develop and maintain vendor and agency networks.
