Job summary

Tri-Counties Regional Center in Santa Maria is seeking a Service Coordinator II (Bilingual Preferred) to deliver person-centered consultation, advocacy and coordination for people with developmental disabilities requiring involved support.

You will conduct IFSP/IPP planning, participate in IDT meetings and IEP/day program meetings, refer to local services, coordinate funded services including behavioral supports, day programs and equipment, and collaborate with families and community partners in

Qualifications

• Bachelor’s degree in a Human Services or related field.
• Minimum of 2 years of social and/or mental health experience.
• A valid California driver’s license and transportation or acceptable substitute.
• Spanish/English bilingual preferred.

Responsibilities

• Conduct strength-based assessments and involve family members in planning and support.
• Design, document, implement and monitor services in a timely manner.
• Coordinate and secure funded services, including behavioral supports and day programs.
• Participate in IDT staffings and attend community meetings as needed.
• Provide information and referrals to community services and resources.
