Job summary

Tri-Counties Regional Center seeks a Service Coordinator to provide person-centered consultation, advocacy, and service coordination for individuals with developmental disabilities who require specialized assistance. You will work in collaboration with families and teams to coordinate plans, secure services, and navigate community resources.

Responsibilities include conducting assessments, involving families in planning, coordinating services, and maintaining relationships with community

Qualifications

• Master’s or Bachelor’s degree in human services or related field with relevant experience.
• 2 years of case management experience in mental health or social work.
• 1 year experience with individuals with developmental disabilities preferred.
• Bilingual abilities in Spanish and English are preferred.

Responsibilities

• Conduct strength-based assessments communicating understanding of family issues.
• Involve individuals and families in person-centered planning and support.
• Design, implement and monitor services and supports in a timely manner.
• Facilitate planning team progress and identify outcomes and needed services.
• Develop and maintain a network of community vendors and agencies.
• Provide information and training about developmental disabilities, services and resources.
