ChristianaCare in Maryland is seeking a Social Worker for our Care Management Triad Team. The role focuses on care coordination, discharge planning, and psycho-social support to facilitate patient progression.

RN Case Manager and Social Worker collaborate with Utilization Management to ensure appropriate status and payer coverage. MSW required; licensed CSW/MSW preferred. We offer competitive benefits and a substantial sign-on for eligible candidates.

Qualifikationen

• Master’s degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.
