ChristianaCare seeks a Social Worker to join our Care Management Triad Team, focusing on care coordination, discharge planning, and addressing psycho-social barriers to ensure safe transitions.

The candidate will hold an MSW or MSS and be licensed (CSW/MSW) where required. Acute care experience is preferred, with access to a broad network of outpatient services and a strong commitment to love and excellence.

Competenze

• Master’s degree in Social Work (MSW) or Master of Social Services (MSS) required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.

Mansioni

• RN Case Manager - manage patient care and drive patient progression and establish a discharge plan.
• Social Worker – resolve psycho-social barriers and supports discharge needs.
• Utilization Management – review patient status for appropriateness and anticipated payer coverage.
